1) Please fill in the following requested data. Be sure to enter the tournament entry deadline, tournament start date, and tournament complete date in the following format:
year (ie. 2006)-month (ie. 12 for December)-day (24 for the 24 th day)
2004-12-15 = Dec. 15, 2004
Be sure to check the date formats!!!
2) Once your data has been entered correctly, click the submit button. A multipage Licensing Agreement should be displayed. Please read the Licensing Agreement very carefully. Print and sign one copy, then mail a completed and signed Licensing Agreement to Super Stars National office, or contact us via e-mail if you prefer to License using a credit card. You will still need to sign the last page and fax a copy to the National office for expedited service. We will review the submitted application dates and locations and approve it within 24 hours providing there are no other previously sanctioned tournaments in your area to compete with. Once you have received a countersigned Contract final page, your tournament is licensed and will be posted on our National Website and many other marketing tools will be made available to make your qualifying tournament a huge success!
